At your appointment, you will be asked to fill out a Health History. If you would like, you can print and fill out the health history form ahead of time and bring to your appointment.

Medical History for Abortion – Medical history for abortion visits (in-clinic and medication abortion).
Medical History Assigned Female at Birth (AFAB) – Medical history if assigned female at birth and being seen for a NON-abortion related visit.
Medical History Assigned Male at Birth (AMAB) – Medical Hx if assigned male at birth, can be completed for any type of visit.
